11/27/2023 EnchantaVaughn Approved QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Ms. Broadnax verbalized to QMHPA that she is was in a good mood because she gets to see QMHPA during the morning. QMHPA informed Ms. Broadnax that this session was going to focus on decreasing and cope with feelings of depression evident by stabilized mood, increased interactions and verbalizing positive feelings. Ms. Broadnax did not present any symptoms of SI or HI at the time of the session being conducted. QMHPA began the session by inquiring about Ms. Broadnaxâs current thoughts, feelings and emotions. QMHPA reviewed with Ms. Broadnax appointments and her method of scheduling. QMHPA encouraged Ms. Broadnax to notify her QMHPA of appointments to assist her with transportation. QMHPA discussed with Ms. Broadnax about identifying triggers that causes depression as well as frustrations. QMHPA engaged Ms. Broadnax in a mental health skills building activity. QMHPA explained to Ms. Broadnax that the purpose of this activity was to explain to Ms. Broadnax improving peer relationship by listening, create two options then reply instead of saying the first thing that comes to mind. QMHPA discussed with Ms. Broadnax about learning and using coping skills when interacting with people throughout the community and using better judgement when meeting new people. QMHPA assisted Ms. Broadnax with an exercise to help control her aggressive behaviors. QMHPA demonstrated with Ms. Broadnax a group of words and Ms. Broadnax has to say the first thing that comes to her head. QMHPA explained the same words to Ms. Broadnax in a different order but allow some time, think then answer the best decision that comes to your mind. Ms. Broadnax appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Broadnax displayed appropriate behaviors in a structured setting. Ms. Broadnax appeared to be organized and keeps track of all of her appointments; however, she was frustrated about the transportation. Ms. Broadnax discussed during the session that she do not like people who think they know it all or disrespectful. Ms. Broadnax said she has always been the person who will say how she feels and keep it real. Ms. Broadnax admitted to saying the first thing that comes to her mind without any thought. Ms. Broadnax provided active participation in the exercise and displayed positive emotions. Ms. Broadnax said she was able to recognize the difference in her thoughts when she thinks about more options. Ms. Broadnax said when she is frustrated or angry she does not have time to think and that is what she is scared of most of the times because she does have a mental disability.
